Be Prepared
What to Bring to Your First Visit
Having the right documents ready helps us spend more time on your care and less time on paperwork.
- Valid government-issued photo ID
- Insurance card(s)
- Current medications in their original bottles
- Medical records from your previous provider (if available)
- List of known allergies
- Any recent lab results or imaging
- Referral form (if required by your insurance)
- Payment method for copay or fees
